Dear Parent/Guardian:

Influenza, also known as the flu, is a serious illness that affects people of all ages.  One of the best ways to prevent the flu is through vaccinations.  We are pleased to again join with the Maryland Partnership for Prevention (MPP) to offer flu vaccinations at our school. New this year, you may give permission for your child to be vaccinated using (1) an online form at www.vaccineconsent.com or hard copy given to your student.

We urge you to protect your family from the flu. The Centers for Disease Control and Prevention (CDC) recommends that everyone ages 6 months and older get a flu shot. Please visit www.cdc.gov/flu/index.htm for more information about the flu vaccine. The clinic at our school will be held on October 25, 2019, from 7:30 am to 11:00 am.  We will send a notice home the day he/she has been vaccinated.

 If you want your child to be vaccinated, please:

1. Fill out every section of the attached Consent Form OR complete it online at   www.vaccineconsent.com.  If you fill out the paper consent, write neatly and use ink.  Only fill out the hard copy consent or the online consent. 
2. Do NOT fill out both. Use your insurance card to fill out your insurance information accurately.  Your insurance company will be billed.  There is no co-pay or deductible.  You will NOT be charged by us or your insurance company
3. Read the Vaccine Information Statement (VIS) and talk to your health care provider or the school nurse about any questions you have.
4. Return the consent form no later than September 21, 2019.

Estimado padre/tutor:

La influenza, o gripe, es una enfermedad grave que afecta a personas de todas las edades. Una de las mejores maneras de prevenir la gripe es con la vacunación. Tenemos el agrado de asociarnos a Maryland Partnership for Prevention (MPP) para ofrecer la vacunación antigripal en la escuela de su hijo.

Le recordamos que se trata de una vacuna antigripal inyectable. Esto significa que se inyectará en el brazo de su niño a través de una pequeña aguja. La vacuna contra la gripe es muy segura. La clínica estará en la escuela de su hijo en 25 de Octubre.

Enviaremos a casa una notificación con su hijo el día en que sea vacunado.  
Si desea que su hijo reciba la vacuna, le pedimos lo siguiente:
    1.    Llene todas las secciones del formulario de consentimiento adjunto o complételo en internet, en el sitio web que se indica debajo de estas instrucciones.
    2.    Consulte su credencial del seguro para completar con precisión la información de su seguro. Se facturará a su compañía de seguro. No se cobran copagos ni deducibles.  Usted NO deberá pagarnos nada a nosotros ni a su compañía de seguro.  
    3.    Lea la Declaración de Información sobre la Vacuna (VIS) y consulte a su médico o al enfermero escolar si tiene alguna pregunta.
    4.    Entregue el consentimiento antes del 21 de Septiembre.
